Key Points
GOLD advocates active case-finding, not general-population spirometry screening. Consider dx if dyspnea/chronic cough or sputum w/ hx of risk factors. Postbronchodilator FEV1/FVC <0.7 required for dx; if ratio 0.6 to 0.8, repeat on separate occasion to confirm dx. Use GOLD ABE Assessment tool. Focus on sx burden, exacerbation hx. Pre-COPD/PRISm (Preserved Ratio Impaired Spirometry) = pt w/ sx/emphysema and physiologic abnormalities w/o airflow obstruction but is at risk, though not all will develop obstruction.
