A 2022 meta-analysis of 49 studies found that berberine significantly reduced all of the following EXCEPT:
inflammatory markers
lipid profile
obesity parameters
systolic blood pressure
fasting blood glucose balance
You are correct. A meta-analysis of 49 studies published last year in Frontiers in Nutrition found that lipid profile, fasting blood glucose (FBG) balance, obesity parameters, and systolic BP were improved with berberine supplementation. However, no significant changes were observed in liver enzymes or inflammatory markers.

The optimal dose of berberine was 1 g/day for trigylceride, total cholesterol, and weight, 1.8 g/day for insulin and homeostasis model assessment-insulin resistance, and 5 g/day for HDL-C. The most effective duration was 40 weeks for FBG and 50 weeks from beginning of berberine supplementation for diastolic BP and waist circumference.

All of the following are contraindications or cautions for prescribing semaglutide, except:
family hx of thyroid tumors
hx of pancreatitis
type 1 diabetes
major depressive disorder
diabetic retinopathy
You are correct. Major depressive disorder is neither a contraindication nor a precaution for semaglutide.

Semaglutide, however, is contraindicated in pts with a personal/family Hx of medullary thyroid CA, in pts with multiple endocrine neoplasia syndrome type 2, in pts with a hypersensitivity reaction to semaglutide, in patients with T1DM, and in breastfeeding patients (PO form).

More info on contraindications and cautions for semaglutide is available at epocrates.
(epocrates)—Pitolisant (Wakix) is a histamine-3 (H3) receptor antagonist/inverse agonist indicated for the tx of excessive daytime sleepiness in adult pts w/ narcolepsy. Pitolisant promotes wakefulness by increasing histamine levels in the brain; therefore, centrally acting H1 receptor antagonists may reduce pitolisant’s effectiveness and should be avoided. Conversely, H1 antagonists that don’t cross the blood-brain barrier aren’t expected to alter pitolisant efficacy.

Which H1 receptor antagonist may be used in combo w/ pitolisant?
loratadine
clomipramine
diphenhydramine
mirtazapine
imipramine
You are correct. Loratadine antagonizes peripheral histamine H1 receptors. Therefore, it’s not expected to decrease pitolisant efficacy.

In contrast, clomipramine, diphenhydramine, imipramine, and mirtazapine all have centrally acting H1 receptor antagonist properties and should be avoided w/ pitolisant.
